Providence is calling an experienced Registered Nurse to be a Patient Placement Navigator at Providence Health & Services in Olympia, WA.
We are seeking a Registered Nurse - Patient Placement Navigator to be responsible for high level assessment and quick initiation of level of care decisions for patients requiring admission or transfer to an inpatient setting. The Patient Placement RN's goal is to provide high quality, individualized, cost effective placement of patients; to prioritize and make decisions for timely transfers based on acuity, diagnosis, bed availability, and unit staffing. The Patient Placement RN collaborates with physicians, administrative supervisors, Clinical Managers and/or charge nurses, admitting, and others as needed to define and achieve accurate admission and placement.
This is a 0.9 FTE (36 hours per week), 12-hour, day shift position.
In this position you will:
Communicate effectively with referring and admitting physicians to determine patient level of care and requirements.
Review all emergent, urgent, and elective admissions.
Identify appropriate admitting physician service for requested admission.
Coordinate with accepting physician regarding patient diagnosis and placement requirements.
Act as resource to Patient Placement Specialist regarding clinical assessment, level of care, and triage of admission requests especially in utilizing limited resources (beds, staff etc.).
Consult with charge nurses regarding patient special care needs, isolation issues, and unit's ability to care for patients within unit's scope of service and bed availability.
Collaborate with medical staff, Administrative Supervisor, Clinical Manager, Charge Nurse, and patient transport staff regarding patient placement requirements.
Coordinate critical care patient placement with transfers to and 'inhouse' transfers from the CCU.
Review blocked and flagged beds daily to complete daily closed bed report with bed numbers and reason for closure.
Required qualifications for this position include:
Nursing Degree from an accredited school of nursing.
Current Washington State Registered Nurse License.
Current Basic Life Support for Health Care Providers certification issued by the American Heart Association.
Three years of RN experience in an acute care (hospital) setting.
Preferred qualifications for this position include:
Bachelor of Science degree in Nursing (BSN).
